Once your portal is active, you will even have the ability to access everything on your mobile device through the Healow App.

From the U.S. Department of Health & Human Services
What is the HIPAA notice I receive from my doctor and health plan?
Your health care provider and health plan must give you a notice that tells you how they may use and share your health information. It must also include your health privacy rights. In most cases, you should receive the notice on your first visit to a provider or in the mail from your health plan. You can also ask for a copy at any time.
Why do I have to sign a form?
The law requires your doctor, hospital, or other health care provider to ask you to state in writing that you received the notice.
The law does not require you to sign the “acknowledgement of receipt of the notice.”
Signing does not mean that you have agreed to any special uses or disclosures (sharing) of your health records.
Refusing to sign the acknowledgement does not prevent a provider or plan from using or disclosing health information as HIPAA permits.
If you refuse to sign the acknowledgement, the provider must keep a record of this fact.
What is in the Notice?
- The notice must describe:
- How the Privacy Rule allows provider to use and disclose protected health information. It must also explain that your permission (authorization) is necessary before your health records are shared for any other reason
- The organization’s duties to protect health information privacy
- Your privacy rights, including the right to complain to HHS and to the organization if you believe your privacy rights have been violated
- How to contact the organization for more information and to make a complaint
When and how can I receive a Notice of Privacy Practices?
- You’ll usually receive notice at your first appointment. In an emergency, you should receive notice as soon as possible after the emergency.
- The notice must also be posted in a clear and easy to find location where patients are able to see it, and a copy must be provided to anyone who asks for one.
- If an organization has a website, it must post the notice there.
- A health plan must give its notice to you at enrollment. It must also send a reminder at least once every three years that you can ask for the notice at any time.
- A health plan can give the notice to the “named insured” (subscriber for coverage). It does not also have to give separate notices to spouses and dependents.

Please remember that patients with an HMO health insurance plan must have Premier Family Health or Dr. Vincent Apicella, DO listed as their Primary Care Provider (PCP). If a patient’s insurance card has another provider’s office listed, we will not be able to bill their insurance company for the visit.
Patients who switch insurance companies should notify us as soon as their plan takes effect.

Telemedicine, in the form of virtual visits, is not a new concept in medical care. In fact, various forms of remote visits have been in use since the 1960’s. Today, due to rapidly advancing affordable technology, Televisits have become an important tool for healthcare providers allowing us to offer service to our patients when in person visits are not practical or possible.
What do you need to have a virtual visit with a provider?- An iPhone, Android phone, Desktop or Laptop Computer (Mac or PC)
- Camera.
- Speaker.
- Microphone.
- Secure reliable internet connection with a minimum speed of 2MBPS

OUR HISTORY
Dr. Apicella and Dr. Bago began their medical training at Nova Southeastern University in August of 1997. They shared many classes together and began dating in January, 1998. After earning their medical degrees, they both entered the Family Medicine Residency Program at Wellington Regional Medical Center where they became a well-respected duo among their peers and colleagues. Through their hard work and determination, they were chosen as Chief and Co-Chief of their residency class in the Family Medicine Program. It was during this time, that they began to formulate the concepts of practice that would become the foundation of the family practice they would later create. They married in 2001.
Premier Family Health, PA was established in 2004. In July of that year, Dr. Apicella and Dr. Bago cared for their first patients in an office on the campus of Wellington Regional Medical Center. It wasn’t long before they built out their own office space in their current location, the Wellington Reserve, with the vision of creating the unique model of health care that exists today.
From the very start, Dr. Apicella and Dr. Bago set out to establish a medical practice with a strong focus on wellness and prevention. The physicians knew that empowering patients through education and motivation was key to helping them achieve their health and wellness goals. It was their goal to develop Premier as a Medical Home for patients, offering many needed healthcare services at one location. This innovative model of healthcare delivery, they were certain, would add great value with regard to providing convenience, and continuity of care to their patients.
Today, Premier is one of the most progressive medical practices in Palm Beach County. A Patient-Centered Medical Home is accountable for meeting a large majority of a patient’s healthcare needs including prevention and wellness, acute and chronic care. Providing comprehensive care requires a team of providers that may include physicians, nurse practitioners and physician assistants.

A TEAM APPROACH
At Premier, a team of professionals is dedicated to coordinating your care. Should your provider-of-choice not be available to see you, please request to see his or her care-team partner. This may be a nurse practitioner or physician assistant working closely with your provider.
ALLWAYS ON CALL 24/7
Premier provides extended access to medical care beyond normal business hours. Medical attention is available through Premier’s Urgent Care Center open Monday through Friday from 8:00am to 8:00pm, and on Saturday from 9:00am to 4:00pm. Urgent care services are available both as same-day appointments or walk-in visits. A Premier healthcare professional is always on-call. A Premier healthcare provider is on-call after hours for urgent matters. Please call 561-798-3030 and follow the prompts to reach the on-call provider.
ACCESS YOUR INFO
Access your personal health information through a secured Patient Portal. Through the Patient Portal, you will be able to request appointments, referrals and medication refills. You may also access your health records, send an email to your provider and find self-help materials. Our staff members can assist with activation once you have provided an email address.
